Does anyone know where I can find a printable rebate form?? My daughter threw mine away

BUY IT! BUY IT! BUY IT!
We’ve lived in a HARD WATER county in Kansas for almost six years now. Downside? Calcium build-up, especially shower/faucet heads, ice maker water line & dishwasher water lines/rotating arm that disburses the water. The powdery film & spots left on all types of dishes was horrible, but our silverware, we thought, was RUINED! I cannot describe the look or feel, other than to say we would only use after hand washing or rinsing what we needed beforehand.
A few months ago, My husband came across OxyClean Dishwashing Boost (‘Peelie’ attached for refund of purchase price – $5/7 oz. & $8/13 oz.). We bought it.
First use – we topped off rinse agent receptacle with OxyClean (still about half full of Jet Dry), added two squirts into bottom of dishwasher, as suggested on label for tougher problems, ran on ‘Heavy/Pots & Pans’ and decided to not run Heated Dry portion, rather run a second time opting on Normal w/Heated Dry. NO additional detergent or OxyClean but felt another round of the cycles certainly couldn’t hurt!
RESULTS WERE NOTHING SHORT OF A MIRACLE!
NO FILM – NO SPOTS – NO POWDERY COATING
It took about three weeks to ‘revive’ everything. Some items required a few trips through, but by the time we were finishing our first bottle. The transformation was complete!
Nowadays, we run Normal Wash/Heated Dry once. Our rinse agent remains OxyClean and we still use the two squirts. We dwithout about three consecutive loads without the squirts, and found that by the third load, we could see cloudiness returning and the shine was diminished. So, we are back on the added squirts. A SMALL price to pay, in our opinion.
I cannot imagine ANYONE being dissatisfied with their results. This is one household that never runs out of OxyClean Boost for Dishwashers!

I found the rebates at Wal-mart here in NWA, too.
